Dear EPCS families and staff:
The need to keep our school community aware of their Covid-19 and variants status is paramount. The Community Outreach Program will continue to partner with US Medical PPE, the mobile testing unit that we began using earlier this year. Continuing to offer to test our students, their families and staff will assist in all of our well-being.
The date and time for the next testing will be as follows:
Tuesday, February 1, 2022, from 3:00 – 7:00 (first come first attended to)
**Please note if you participated in the last session you are already registered and do not need to re-register****
Each person who wants to get tested must register using the link provided below. (QR code is also provided)
You are asked to upload just 1 piece of photo identification you can use
State-issued ID
Passport
Driver License
If you do NOT have an ID for your child please enter a zero (0) front and back when asked to upload the image
If you have health insurance you are asked to upload your insurance card.
If you DO NOT have health insurance you CAN still be tested without being charged please still register and select uninsured.
All parents or guardians must fill out the consent form to have the children tested and bring them at the time of testing.
